Renal Angiomyolipoma: Diagnostic Difficulty in Imaging Studies

Abstract
Purpose: Renal angiomyolipoma (AML) is a benign tumor composed of mature adipose tissue, thick-walled blood vessels and sheets of smooth muscle. Because many of these tumor contain fat element, they lend themselves well to fat-sensitive imaging, such as ultrasonography (US), CT and MRI. But diagnostic difficulties arise in predominantly myocomponent lesions, AML with hemorrhagic complications and coexisting renal cell carcinoma. We present 5 cases of unusual radiological appearance of AML, which diagnosed by special stain and pathologic finding.

Materials and Methods: From June 1998 to November 2000, authors reviewed their personal series, US and CT or MRI findings of 5 cases with atypical features of renal AML.

Results: Five patients underwent surgery to differentiate renal malignancy showed predominantly muscular, vascular components and scant or absent fatty cells on histology. Renal AML was comfirmed by immunohistochemical stain with a panel of antibodies including Desmin, Actin and HMB-45.

Conclusions: To avoid functional kidney loss in benign renal pathology, nephron sparing surgery is necessary in equivocal small renal mass.
